Our clients will be asked to sign their own terms and conditions. Below you can find the standard wording for the T&Cs.

This is an agreement between you and Neurodiverse Online. Please read this Assessment and Therapy Agreement carefully and ask any questions before indicating your consent by clicking the button at the end. It contains important information about your assessment and therapy and the terms under which you receive it. There are three main sections: Terms and Conditions Assessment and Therapy Contract Data Protection and Privacy

 

Assessment and Therapy Agreement for Clients of NeuroDiverse Online

This is an agreement between you and NeuroDiverse Online.

Please read this Assessment and Therapy Agreement carefully and ask any questions before indicating your consent by clicking the button at the end. It contains important information about your therapy and the terms under which you receive it. There are three main sections:

  1. Terms and Conditions

Terms and Conditions

Fees and payment method

The fee for your sessions will have been previously agreed by email. For clients paying privately themselves, payment is taken in advance at the time of booking. Payment should be made via GoCardless Instant Pay or Stripe depending on the platform you booked your appointment. For full diagnostic assessments, payment can be made in advance in full by GoCardless Instant pay or BACs transfer. Where payment by instalments has been agreed, it should be made via GoCardless direct debit. If none of these options are possible, then we can take payment via Square by prior arrangement. Please note that if you default on your instalment payments, we will stop the assessment process and cancel any future appointments. This means that you will not receive your diagnosis and report. We will take steps to recover our fee via a debt recovery agency. Clients covered under health insurance will have separate arrangements which will have been negotiated previously. When claiming under health insurance, please note it is your responsibility to check the treatment being provided is covered under your policy. If it is not, you may be personally liable for the cost of this treatment and our fees. It is your responsibility to pay any excess on your medical insurance policy and NeuroDiverse Online will send you an invoice for monies owing.

Please note that we reserve the right to delay receipt of your assessment report should monies be owed to us. Fees are reviewed on 1st January each year. You will be notified a month before if your fee is to change.

Cancellation policy

If you must cancel or reschedule an appointment, 48-hour advance notice is required, otherwise you will be held financially responsible. We are happy to refund your fees if you choose to cancel your appointment with 48-hour advance notice, but there may be a small charge to cover the costs of our bank processing fees. This is typically 2-4%.

Should you cancel or miss an appointment with less than 48 hours’ notice, this will result in being charged the full fee for your missed appointment. Where a package has been purchased, or insurance policy utilised, that session is ‘lost’.

Phone/video sessions are treated the same as face-to-face sessions. If you are late getting on the phone, are unable to talk at the scheduled time, your battery has died, you are unable to access a confidential place to talk, or any other variable that prevents you attending our session please know that you will be charged for the session. Please make the necessary arrangements you need to be available and present for your session.

Your therapist will endeavour to give you as much advance notice as possible of holidays or of any need to re-schedule a future appointment.

In the unlikely event your therapist is incapacitated and is unable to contact you, a member of our team will contact you. They are bound by the same rules of confidentiality as your therapist is, and will only access your contact details in an emergency.

For people seeing our therapists online: the logistics of online assessment and therapy

For phone/video sessions, your therapist will call you at the scheduled time or send you a link for the secure video session. Your therapist will expect that you are available at the scheduled time and are prepared, focused and engaged in the session.

Your therapist will call you from a private location where they are the only person in the room. You also need to be in a private location where you can speak openly without being overheard or interrupted by others to protect your own confidentiality. If you choose to be a in a place where there are people or others can hear you, your therapist cannot be responsible for protecting your confidentiality. Every effort must be made on your part to protect your own confidentiality. we suggest you wear a headset or earbuds to increase confidentiality and this will also increase sound quality of the sessions.

Please know that your therapist cannot guarantee the privacy or confidentiality of conversations held via phone, as phone conversations can be intercepted either accidentally or intentionally.

Please ensure you reduce all possibilities of interruptions for the duration of the scheduled appointment.

To minimize the chances of technological glitches interrupting our session, your therapist will utilise modern computer equipment, close all other applications and browser windows, where possible use a wired Ethernet connection, and headphones/earbuds: all of these things are known to maximize the quality and stability of the call. We suggest you do the same. The most common reason for calls dropping out is reliance on wireless signal. While video calling on a mobile phone or tablet is convenient, it relies on wireless signal. Accessing a desktop computer with wired Ethernet is generally more reliable.

Connection loss

If the connection is lost during a session, your therapist will call you to troubleshoot the reason for the lost connection. If you are unable to reach each other due to technological issues, your therapist will attempt to call you 3 times. If they cannot reach you, they will remain available to you during the entire course of the scheduled session. Should you contact your therapist back and there is time left in the session, it will be continued.

If the problem occurs in the first 20 minutes of the session for whatever reason, and it is not possible to re-establish the call, your therapist will cancel the session and they will offer you another full session free of charge.

If the reason for a connection loss is on your part and the session has progressed beyond 20 minutes before technological glitches were experienced e.g. battery dying, bad reception, etc, the full fee for the session will be charged. If the loss for connection is a result of something at your therapist’s end, you can either complete the session via phone, or plan an alternate time to complete the remaining minutes of the session.

About a Psychological Assessment

 We will be helping you to clarify the nature of your problems and provide our opinion of whether you qualify for a diagnosis of a neurodevelopmental condition.

 We will then provide recommendations so that you can cope better with your problems and feelings, as well as realise your full potential. One of the recommendations may involve therapy, with the therapist who assessed you or someone else.

 Regularly scheduling and attending appointments will be important to get the most out of the assessment and achieve the clarification and gains you seek.

 To seek greater clarity about the nature of your problems we will administer questionnaires. Please ensure you complete these promptly.

About Therapy

Therapy is likely to help you understand more about yourself and others. Your therapist will be helping you to develop skills and understanding so that you can cope better with your problems and feelings, as well as realise your full potential.

While your therapist cannot provide a guarantee, attending therapy is likely to help you feel better and produce beneficial results.

 Regularly scheduling and attending your appointments will be important to get the most out of therapy and achieve the gains you seek.

 There are various ways you may know that therapy is working: Your therapist may administer questionnaires and you are likely to see an improvement on your scores, or you may notice differences in how you feel or cope with problems. Many people report improved relationships, better decision making and a greater sense of purpose.

Our commitment to you

Your therapist will be honest and ethical in all their dealings with you. They will abide by strict codes of conduct (HCPC).

Your therapist will work collaboratively with you, selecting a treatment plan informed by your goals and the best evidence-based practice.

 

Your therapist will use their expertise and experience to guide you in learning new ways of thinking and coping that are designed to help you feel better.

Your therapist will be open to your feedback and to adjust their approach in your work together.

Your therapist may provide you access to written and/or online self-help resources as part of your fee. You are welcome to continue using these after your work has finished.

Your commitment to us

We ask that you be honest and open with your therapist at all times. If you have a concern, please talk to your therapist about it and they will do their best to address it.

Please give your therapist a minimum of 48 hours’ notice if you need to change your appointment. If you are late, give less than 48 hours’ notice or forget your appointment, you will still be charged the usual fee, due to this time having been reserved for you. This will also apply if you are having your sessions paid for by an insurance company.

Please understand that you are responsible for attending your appointment and although we use email confirmation and auto- SMS reminders, we cannot be held responsible in the event of emails or SMS not reaching you, and you do not attend.

If you are unhappy about any aspect of therapy and would like to end your sessions, please talk to your therapist about it. If you still wish to end, then we encourage you to plan a final session following a notice of therapy cessation.

For clients receiving therapy online or by telephone: We ask you to be prepared for sessions by making sure you are in a quiet, private space, having checked the technology is working in good time, and have not recently taken alcohol or drugs.

Please participate actively in the therapy, as much as possible trying out the techniques and completing the home-based tasks you will have agreed with your therapist between sessions.

Your therapist’s approach

All our therapists are qualified Practitioner Psychologists, and are trained in a variety of therapy approaches and qualified to work with clients across the lifespan.

 As with any therapy, the more you put in the more you get out: evidence suggests that therapy is more effective and works faster if the client completes homework tasks between sessions. Your therapist will therefore offer written or electronic materials to utilise between sessions. This is included as part of your fee, but it is entirely your decision whether you take up this extra opportunity or not.

To track your progress, your therapist may ask you to regularly complete questionnaires.

Boundaries and dual relationships

Keeping proper therapeutic boundaries is important so that the therapy you receive is ethical and effective. Therapists having dual relationships with clients is considered by most professional bodies as unhelpful and potentially damaging to the wellbeing of clients. A dual relationship is when a therapist and client engage in two different roles with each other e.g. being a therapist and a friend to a patient, having a business partnership with a patient as well as providing therapy to them, or a therapist having a romantic or sexual relationship with a patient. These relationships can be exploitative and harmful for clients and therefore your therapist will not engage in dual relationships.

We operate on a strictly ‘need to know’ basis and therefore do not divulge confidential information about you to other people. If your therapist is concerned that you may harm yourself, or another person they may need to break the boundaries of confidentiality. They will always try to discuss this with you first.

Your therapist will maintain the boundaries of your relationship by choosing not to interact with their patients on social media.

They will not therefore accept invitations to be a ‘friend’ and will not respond to comments left on Facebook or other social media sites. Please see our Social Media and Communications Policy.

Benefits of online assessment and therapy

You have a greater range of therapists to choose from.

By seeing a therapist online, you don’t have to travel. This saves you time and can be cheaper because you don’t have to pay for transport or parking etc.

If you have health or caring responsibilities, online therapy may be more convenient.  You have greater flexibility about when to see your therapist

 You may be able to see someone quicker and not have to wait.

Limitations of online assessment and therapy

Whilst there are advantages to online therapy, there are also some disadvantages which may affect your experience. They may include:

Body language: The amount of non-verbal communication will be reduced as your therapist will normally only be able to see your head and shoulders.

Technology glitches may interrupt our session. Your therapist will reduce the chances of this by only using the most modern equipment, business-quality videoconferencing platforms like Zoom, Teams or the WriteUpp Video call Platform which are renowned for their stability.

While your therapist will only use encrypted, secure online platforms to protect your privacy and confidentiality, a 100% guarantee cannot be given that a breach may not occur. However, even with in person therapy risks exist e.g. someone seeing you enter the therapy clinic, a conversation being overheard in a poorly sound-proofed room

Data Protection and Privacy

GDPR (General Data Protection Regulation) is the most significant piece of privacy and data protection in twenty years. It took effect on 25th May 2018 and from that date we have been required to ensure that we gain a new data protection and privacy consent from all clients. In it (amongst other things) we confirm what information we hold about you and how we are permitted to use it. Once you have reviewed the information contained in this document please confirm your consent by clicking on the “Consent” button at the end.

This consent form should be read in conjunction with the Privacy Policy on the NeuroDiverse Online website. These include our Website Disclaimer and use of Cookies. In accordance with Article 13 ‘Right to be Informed’ of the GDPR we are bound by law to inform our clients:

Who we are and what service we provide

All our therapists are Registered Practitioner Psychologists with the United Kingdom Health and Social Care Professions Council. They provide psychological therapy to clients as associates of NeuroDiverse Online. NeuroDiverse Online Ltd is a registered company (15067548).

Why we need your personal data

As health professionals we are required to keep records of the work we do. We only collect and store information which is relevant to our therapeutic work together, and which will enhance your care. We are permitted to collect and keep this information by law.

We routinely audit the effectiveness of the service we offer and for this reason retain information about the outcomes of therapy such as scores on the questionnaires you may be asked to complete, or the number of therapy sessions you have. Any reports we write use only anonymised data and are not linked to any identifying details. If you do not wish us to include your anonymised data in this way, please let us know.

How we will use your data within our practice

We use the data we hold about you to contact you about appointments and to take notes about what we have covered together. This ensures continuity of care and means we are able to provide you a better service.

Storage of client material: We use two cloud-based tools to collect and store your personal information and case notes securely: WriteUpp and Microsoft 365. We do not store client identifiable information on computer hard disks.

With your consent, we use two AI scribes to record and make notes on your sessions: NovoNote and Heidi Health.

Payment: NeuroDiverse Online will invoice you directly at the time of booking. Payments will be taken via GoCardless, Stripe or Square.

Video calls: We use Zoom or Teams video calling functionality to conduct our therapy sessions online. All calls are encrypted.  Emails: Please limit email contact to only appointment-related matters as this medium cannot be guaranteed 100% secure.

Appointment scheduling: Initial appointments will be arranged via the email address you provided us when you first got in contact.

We may use Teams Bookings or Calendly to offer clients the option of booking directly into your therapist’s diary.

Text messaging and phone calls: If you wish to use regular text messages or telephone calls to communicate with your therapist, please note that they are not 100% securely confidential. If you choose to use them please restrict their use to making or changing appointments. If you would like a more secure way of texting or phoning your therapist, you can use Signal. This is similar to WhatsApp but more secure. Let your therapist know if you would like to use this option, otherwise they will assume you are happy for us to communicate with you using your regular telephone number.

Questionnaires: We may use NovoPsy ch, MHS, WPS, QGlobal or PAR to administer questionnaires and other psychology forms.

How you can modify the data we hold about you

We routinely share with our clients assessment letters or discharge reports before they are finalised. There is an opportunity to request amendments at that point. If you wish to modify other data that we hold about you, please let us know so we can discuss it further.

How you can access data we hold about you

You have a right to request to see the data we hold about you. Legally we are required to respond within 30 days. Please contact us if you would like to access your data.

How you can you retract consent for us to hold and process your records

NeuroDiverse Online will retain your personal information and health records for 8 years and thereafter they are destroyed. This is in line with UK best practice for adult health and social care records (IGA, 2016). You have a right to retract consent to hold and process your records before that time if you wish. Please let us know if you wish to do so.

How you can raise a complaint

If you are concerned about the healthcare we have provided to you, we encourage you to contact us by calling 0203 198 1024 or email admin@neurodiverseonline.com. If you feel your therapist has done something harmful or unethical and you do not feel comfortable discussing it with them, or with our Director, you may contact the Health & Care Professions Council for registered health practitioners (which includes psychologists in the UK). Regardless of your place of residence, the applicable law is that of England and Wales and any disputes will be subject to it.

Who we are and what service we provide

All our therapists are Registered Practitioner Psychologists with the United Kingdom Health and Social Care Professions Council. They provide psychological therapy to clients as associates of Neurodiverse Online. NeuroDiverse Online Ltd is a registered company (15067548).

Why we need your personal data

As health professionals we are required to keep records of the work we do. We only collect and store information which is relevant to our therapeutic work together, and which will enhance your care. We are permitted to collect and keep this information by law.

We routinely audit the effectiveness of the service we offer and for this reason retain information about the outcomes of therapy such as scores on the questionnaires you may be asked to complete, or the number of therapy sessions you have. Any reports we write use only anonymised data and are not linked to any identifying details. If you do not wish us to include your anonymised data in this way, please let us know.

How we will use your data within our practice

We use the data we hold about you to contact you about appointments and to take notes about what we have covered together. This ensures continuity of care and means we are able to provide you a better service.

 Storage of client material: We use a cloud-based tool to collect and store your personal information and case notes securely: WriteUpp. We do not store client identifiable information on computer hard disks.

  Payment: NeuroDiverse Online will invoice you directly at the time of booking using GoCardless, Stripe or Square.

 Video calls: We use Zoom or Teams video calling functionality to conduct our therapy sessions online. All calls are encrypted.  Emails: Please limit email contact to only appointment-related matters as this medium cannot be guaranteed 100% secure.

  Appointment scheduling: Initial appointments will be arranged via the email address you provided us when you first got in contact.

We may use Calendly to offer clients the option of booking directly into your therapist’s diary.

 Text messaging and phone calls: If you wish to use regular text messages or telephone calls to communicate with your therapist, please note that they are not 100% securely confidential. If you choose to use them please restrict their use to making or changing appointments. If you would like a more secure way of texting or phoning your therapist, you can use Signal. This is similar to WhatsApp but more secure. Let your therapist know if you would like to use this option, otherwise they will assume you are happy for us to communicate with you using your regular telephone number.

  Questionnaires: We may use NovoPsy ch, MHS or PAR to administer questionnaires and other psychology forms.

How you can modify the data we hold about you

We routinely share with our clients assessment letters or discharge reports before they are finalised. There is an opportunity to request amendments at that point. If you wish to modify other data that we hold about you, please let us know so we can discuss it further.

How you can access data we hold about you

You have a right to request to see the data we hold about you. Legally we are required to respond within 30 days. Please contact us if you would like to access your data.

How you can you retract consent for us to hold and process your records

NeuroDiverse Online will retain your personal information and health records for 8 years and thereafter they are destroyed. This is in line with UK best practice for adult health and social care records (IGA, 2016). You have a right to retract consent to hold and process your records before that time if you wish. Please let us know if you wish to do so.

How you can raise a complaint

If you are concerned about the healthcare we have provided to you, please call us on 0203 198 1024 or

email www.neurodiverseonline.com. If you feel your therapist has done something harmful or unethical and you do not feel comfortable discussing it with them, or our Director, you may contact the Health & Care Professions Council for registered health practitioners (which includes psychologists in the UK). Regardless of your place of residence, the applicable law is that of England and Wales and any disputes will be subject to it.

 

I have read and understood all of the above information, including the accompanying Policies mentioned herein (Audio Visual Recordings Policy available here). I consent to receive psychological services under the terms described in this contract.

1. I understand that the Clinical Director, Claire Larder, her Office Manager, and any associated staff at The Clearly Focused ADHD Clinic, will take all necessary steps to protect my privacy and information. 

2. I understand that The Clearly Focused ADHD Clinic may need to review, use and disclose information about me, my medical/psychiatric diagnosis and treatment to other professionals or care providers. 

3. I give permission for The Clearly Focused ADHD Clinic to share information with my GP, the NHS and / or other Specialists, as required. 

4. I give permission for my GP to share information with The Clearly Focused ADHD Clinic, both mental health and physical, as required. 

5. To the best of my knowledge, my physical and mental health history and status, has been/will be accurately and fully disclosed to The Clearly Focused ADHD Clinic. I acknowledge that the clinic cannot be held responsible should I falsify statements about my health, grossly exaggerate symptoms or purposely omit required information. 

6. It is my responsibility to inform The Clearly Focused ADHD Clinic of any changes in my medical status, psychiatric/mental health status and social status. 

7. I understand that The Clearly Focused ADHD Clinic will perform a full assessment and may request other investigations, as required. I understand that recording these assessments, either as an audio or video recording, without express permission of The Clearly Focused ADHD clinic, eg for training purposes, is strictly prohibited. I understand that not all assessments will lead to a diagnosis. A lack of supporting evidence, a low level of associated impairment or complex co-morbid conditions may make a repeat assessment, at a later date, necessary. 

8. I agree to follow any agreed treatment plan, as discussed with The Clearly Focused ADHD Clinic. I am aware that this may carry a risk of side-effects, withdrawal symptoms and possible other complications at normal doses, but if I do not comply with, or adhere to, the instructions and/or if I exceed the recommended prescribed dosages of medication, the risks will be much greater. 

9. I understand that the clinic cannot be held responsible for me not booking a review or a repeat prescription in a timely manner or running out of medication. Prescriptions are posted within two business days of request/consultation, by Royal Mail First Class Signed For and are valid for only 28 days from signing in the case of methylphenidate, lisdexamfetamine and dexamfetamine.. The clinic holds no responsibility for the services of Royal Mail and any extra prescriptions will need to be paid for, as will any which are lost by the patient or allowed to expire.. I understand that I can pay an additional fee for Royal Mail Special delivery and faster prescription turnaround, if required. 

10. I understand that the clinic cannot accept responsibility for the global ADHD medication shortage. Whilst we check availability daily, there may be occasions where a pharmacy cannot supply a given medication. The clinic’s out of stock medication service can be used in these instances. 

11. I understand that it is at my discretion to stop treatment or investigations, however, if I decide to do so, I understand that this may carry serious risk of side-effects, withdrawal symptoms and/or other complications. 

12. I understand that if I decide to stop treatment or the assessment process, I will be solely responsible for any further side-effects or risks due to premature discharge against medical advice. 

13. I understand that The Clearly Focused ADHD Clinic is unable to treat patients in crisis, emergency or high-risk situations within, or outside working hours. If my mental health condition deteriorates, and I believe I am in an emergency or crisis situation, e.g., having extremes cardiac type symptoms, having suicidal thoughts, considering harm to myself, others, or to property, or if I am at risk from others, I will inform my relative/carer and contact the Local NHS Crisis Team, go to the nearest A&E, or call an Ambulance or Police on 999 or 111 for immediate support. 

14. I understand that The Clearly Focused ADHD Clinic Cannot guarantee that my GP or school will accept a private diagnosis or request for Shared Care. I understand that the clinic does offer a service to clarify my GP’s position, if required. I understand that, even after using this service, the clinic is not liable for any refusal to accept these, due to a GP change of policy.
